Phyllis was born on August 2, 1934 and passed away on Thursday, September 15, 2016.

Phyllis was a resident of Knoper, Michigan at the time of her passing.

She was married to Robert.

Funeral services will be held 11 a.m. Monday September 19 2016 at Fellowship Reformed Church 6610 36th Avenue Hudsonville with Rev. Shawn Hulst officiating. Interment will be at Georgetown Cemetery. Relatives and friends may meet her family Sunday from 2-4 6-8 p.m. at the Cook Funeral Home West Building 4235 Prairie St. SW Grandville and Monday from 10-11 a.m. at church.
